JOB TITLE:                     Case Manager - Focused on Self-Sustainability
 
JOB STATUS:                  Full Time 35 hours / week
 
REPORTS TO:                 Director of Client Services
 
JOB DESCRIPTION:     Work with Clients to discover support resources, provide Neighborhood House services and establish Financial Wellness to ensure household sustainability.              
 
COMPENSATION:         $18 / hour, Paid Holidays, Vacation and Personal Time Off, No Health Benefits
 
JOB DUTIES:                 Neighborhood House Case Managers are responsible for ensuring clients have access to resources and services needed and establish Financial  Wellness and household sustainability.  Tasks include, but are not limited to:
 

  • Learn and Utilize the Client Services Tracking System.
  • Conduct Assessments and Analyze the Data.
  • Review and analyze the current income and spending including a review of all bank statements.
  • Analyze the current budget and work with the client to develop financial awareness and a sustainable budget moving forward.
  • Collaborate with the client to create and document a plan based on the assessment data and conversations, including targets and assignments to be completed to balance the budget and improve overall wellness.
  • Consistent and timely follow up with each client to ensure targets are being met and the plan is being completed.
  • Timely data entry of all client information and case management services.

Education:                       Four Year College Degree
 
Experience:                     3+ years of Case Management in a social services type setting
                                            Analyzing income/spending and creating a sustainable budget
